Liability & Consent

I acknowledge and confirm that I have answered the questions on page one of this document truthfully and to the best of my ability.

I acknowledge that I am aware of and understand the risks associated with this tattoo procedure, including but not limited to allergic reactions from the pigmentation or anesthetic cream.

I understand that there is a risk of infection from the procedure, especially if I do not take care of the area as advised by my tattoo artist.

I acknowledge and agree that if I experience any issues following the procedure, including those noted above, I will contact my tattoo artist but will be solely responsible for the cost of any medical treatment.

I acknowledge and understand that tattooing will result in a permanent change to my appearance, and no representation has been made to me that I will later be able to remove or alter the results.

I confirm that I do not have any moles or sunburn in the area to be tattooed, and I understand that tattooing over moles is not permitted.

I acknowledge and confirm that I have received the aftercare instructions from my tattoo artist and will follow them.

I am not currently under the influence of alcohol or recreational drugs.

I agree to release, indemnify, and forever hold harmless my tattoo artist from any and all claims, damages, and legal actions arising from or connected in any way to my tattoo procedure.

I have seen and agreed to the design, placement, size, and color. 

I give permission for my tattoo artist to proceed with the tattoo.

Release Form

I hereby grant and authorize, Tattoos By Mattson Masterpiece the unrestricted right to take, edit, alter, publish, post and make use of all photos, videos and/or audio ("the materials") that have been taken of me.  I further grant permission for the materials to be used for any lawful purpose, including but not limited to advertising, promotion, marketing, and packaging, without payment or consideration.

I understand that Tattoos By Mattson Masterpiece may modify or edit the materials at their discretion, and I waive any right to inspect or approve the final images or any finished product where these materials may be used.

I acknowledge that I will not receive any financial compensation for the use of these materials, and that all rights, title, and interest in the images belong to Tattoos By Mattson Masterpiece. I further release Tattoos By Mattson Masterpiece from any claims, demands, or liabilities arising out of or in connection with the use of these materials, including but not limited to any claims for defamation, invasion of privacy, or copyright infringement.

I understand that Tattoos By Mattson Masterpiece holds the ownership and copyright to the materials mentioned above and has the exclusive right to reproduce, distribute, display, and create derivative works based on these images. I agree not to reproduce or authorize the reproduction of these images in any form without the express written consent of Tattoos By Mattson Masterpiece.

This permission shall continue indefinitely and applies to all languages, media formats, and markets.

I hereby release and hold harmless Tattoos By Mattson Masterpiece, and its agents, representatives and employees, from any and all claims or actions arising out of, or in connection with the use of the materials.
